Responsibilities:

• Coordinating total patient care by conducting comprehensive health and psychosocial evaluations, monitoring patients' conditions, promoting sound preventive practices, coordinating services and performing teaching and training activities.
• Evaluating effectiveness of nursing services to patients and families
• Consulting with attending physicians for change-of-Care Plans, and coordinating care accordingly with the Director of Nursing.
• Participating in development and periodic revisions of physicians' Plans of Treatment, and processing change orders as needed.
• Covering and filling in for other nurses as assigned by Director of Nursing.
• Providing professional, consistent, skilled nursing care as outlined in nursing care plan.

Minimum Qualifications:

• RN/LPN license required.
• Demonstrated competence in interpersonal relationships, verbal and written communication.
• Skills in Microsoft Office and EMR software.
